Care Navigator​

Duties

• Conduct comprehensive assessments of clients’ needs and circumstances.

• Develop, implement, and monitor individualized case management plans.

• Provide crisis intervention and management to clients in distress.

• Collaborate with community resources, healthcare providers, and social services to ensure holistic support for clients.

• Maintain accurate documentation of client interactions, progress notes, and case files.

• Supervise and mentor junior staff or interns as needed.

• Facilitate access to addiction counseling, behavioral health services, and child welfare programs.

• Support workforce development initiatives by assisting clients in job readiness training and employment opportunities.

• Advocate for clients’ rights and needs within various systems.
   

Requirements

• Bachelor’s degree in Social Work, Psychology, or a related field is preferred.

• Experience in crisis management and intervention is highly desirable.

• Knowledge of addiction counseling principles and behavioral health practices.

• Familiarity with child welfare policies and procedures is a plus.

• Strong supervisory skills with the ability to mentor others effectively.

• Excellent communication skills, both verbal and written.

• Ability to work collaboratively in a team-oriented environment while maintaining client confidentiality.

• Proficiency in case management software or similar tools is beneficial.

• A commitment to ongoing professional development in relevant areas such as childcare or workforce development.
