We are seeking a dedicated and compassionate Care Navigator to join our team. The ideal candidate will play a crucial role in supporting individuals and families and navigating complex systems of care. The Care Navigator will work collaboratively with clients to assess their needs, develop personalized care plans, and connect them with appropriate resources.
Care Navigator
Duties

Conduct comprehensive assessments of clients’ needs and circumstances.

Develop, implement, and monitor individualized case management plans.

Provide crisis intervention and management to clients in distress.

Collaborate with community resources, healthcare providers, and social services to ensure holistic support for clients.

Maintain accurate documentation of client interactions, progress notes, and case files.

Supervise and mentor junior staff or interns as needed.

Facilitate access to addiction counseling, behavioral health services, and child welfare programs.

Support workforce development initiatives by assisting clients in job readiness training and employment opportunities.

Advocate for clients’ rights and needs within various systems.
Requirements

Bachelor’s degree in Social Work, Psychology, or a related field is preferred.

Experience in crisis management and intervention is highly desirable.

Knowledge of addiction counseling principles and behavioral health practices.

Familiarity with child welfare policies and procedures is a plus.

Strong supervisory skills with the ability to mentor others effectively.

Excellent communication skills, both verbal and written.
